A shadow market for unapproved weight-loss drugs is being driven by TikTok fitness influencers and sellers on messaging platforms, based on new investigations.

The experimental drug, which remains in research studies, is being advertised by social media influencers for its supposed weight-reducing effects. Followers are urged to contact these accounts privately for supplier links, while also obtaining discount codes.

Separately, a flourishing trade exists on Telegram, where groups with thousands of members share “before and after” images, administration instructions and assertions of next-day British shipping. Sellers market the substance there before directing buyers to WhatsApp for transactions.

Retatrutide is an investigational injectable developed by American pharmaceutical company Eli Lilly. The treatment targets three gut hormones – GLP-1, glucose-dependent insulinotropic polypeptide and glucagon. Early trials indicate it could help patients lose up to 25% of their body weight. Referred to as the “triple G”, the drug is being hailed digitally as the successor to Ozempic.

Medical Experts Voice Concerns

Nevertheless, doctors and pharmacists have warned that the sale of what remains an unlicensed medication in the UK is a cause for concern and could result in individuals injecting substances of uncertain source.

Several experts said the demand for the experimental drug could be linked to the rising price of approved weight loss products such as Mounjaro.

Selling retatrutide on TikTok or messaging apps is illegal in the UK, as it is an unlicensed medication that cannot be marketed or distributed for human use under the Human Medicines Regulations 2012.

Several content creators try to circumvent the regulations by calling to the substance as a “research chemical” or describing it as “not for human consumption”. Retatrutide is not a restricted drug, but advertising and supply can still breach pharmaceutical, advertising, and customer regulations, potentially leading to penalties, prosecution or prison.

Within minutes of joining one community, a vendor posted contact information, where most purchases take place. The vendor provided an guidebook and promised delivery within two days. When questioned whether the product was genuine, one vendor said: “You can’t be sure.” Another shared a photo of a supposed laboratory certificate. Not one asked about body mass, health issues or past usage.

Within these groups, posts show black plastic parcels with Royal Mail labels, claiming to include fresh supplies. One message stated: “It took eight of us to load them this morning.”

Buyers can select between powder vials, mixed at home with purified liquid, or pre-filled pens usually priced at 160 pounds for 20 milligrams. When questioned how much time a pen should be used, sellers give contradictory answers varying between a few weeks to “depends on your body”.

Service Oversight and Safety Measures

A spokesperson for the messaging service stated it regularly monitored harmful content on its service including content that promoted the “misuse of controlled medications”. The company noted that it removed “millions of pieces of harmful content each day”.

Healthcare Expert Warnings

“People are administering substances they can’t verify. Although the science behind this drug is sound, these digital products may not include what they advertise,” stated a pharmacy expert, the director of pharmaceutical services at a pharmacy service.

Dr Crystal Wyllie of an online medical service connected the surge in demand to current price rises for approved weight-loss medications such as Mounjaro. “With costs rising, patients are using online platforms. But fake treatments are extremely dangerous. You can’t be certain what’s inside or whether the environment they were made in was sterile,” she explained.

Her staff has already encountered individuals harmed by self-injection from unconfirmed sources. “We understand the frustration, but purchasing from Telegram or WhatsApp is gambling with your well-being,” said Wyllie. “These substances could be contaminated or improperly measured.”

A spokesperson from the pharmaceutical company said retatrutide was in third stage research studies and had not been reviewed or authorized by any regulatory agency anywhere in the world. “Consequently at this time no one can market [it] for personal consumption,” they explained, noting that any product “misleadingly claiming itself as a company investigational product not yet authorized … may expose patients to potentially serious medical dangers”.
